You must file an originally executed proof of claim. If you would like a copy of your claim returned to you as proof of receipt, please enclose an additional copy and a self-addressed postage-paid envelope.
Please note that if you previously submitted a substantially complete Hair Straightening Claim electronically, you do not need to submit a Hair Straightening proof of claim in paper form.
TO COMPLETE AND/OR AMEND A PREVIOUSLY-FILED (INCLUDING A PARTIALLY-COMPLETED) HAIR STRAIGHTENING PROOF OF CLAIM
Pursuant to the Bar Date Order, claimants that filed Hair Straightening proofs of claim electronically (including certain partially-completed Hair Straightening proofs of claim) on or before March 23, 2023 at 4:00 PM ET were provided with the opportunity to access the voting portal to vote on the Debtors’ Plan.
If you filed a partially-completed Hair Straightening Claim on or before March 23, 2023 at 4:00 PM for the purpose of voting (or otherwise), you may use this portal to submit a fully-completed claim and to upload all the necessary supporting documentation. Please note, if to-date you submitted a partially-completed Hair Straightening Proof of Claim form only (for purposes of accessing the voting portal or otherwise), you must submit a substantially-completed amended Hair Straightening Proof of Claim on or prior to April 11, 2023 at 5:00 p.m., prevailing Eastern Time in order for your proof of claim to be considered properly and timely filed. For the avoidance of doubt, you will not be able to re-access a previously-submitted claim to make amendments or upload attachments; rather, you must file a new claim and indicate in the appropriate field (as prompted) that your new claim is amending your previous claim.
